SSO is committed to a diverse workforce, which is a reflection of our clients and the people they serve. **Choice Counselor - State of Vermont (Remote)** **Overview:** **This is a remote call center position and requires private space and adequate Wi-Fi connectivity during all active work hours.** Choice Counselors support older Vermonters in the community to stay as independent as possible in the environment of their choice by promoting health, rights, independence, and economic well-being. The purpose of this position is to make and receive calls from disability clients and document basic information about their cases, and provide information about services available to the client to support them in making choices related to their care. **Duties and Deliverables:** The duties of this position include: + Assess the needs of those individuals who have made initial contact with CVCOA, requesting information or assistance, using the Independent Living Assessment, a person centered Options counseling intake, and the "my options" plan. + Provide Options Counseling to help consumers, and their advocates, direct their own decision-making and planning. + Provide consumer-directed and interactive short-term decision-support to consumers, family members, guardians and/or caregivers who are planning for long term services and supports. + Gather information necessary to ensure services and support options provided and referrals made are viable and appropriate based on the consumer's preferences, needs, and goals, as determined by the consumer. + Assure all consumers electronic and paper records are maintained according to ADRC and CLW documentation procedures and standards. Track and gather data and information necessary to ensure required reports are accurate and complete. Work closely with supervisor to actively pursue and engage in outreach and marketing opportunities on ADRC and Options Counseling. + Updating case records and completing documentation requirements. + Participate in programmatic evaluative and quality assurance activities. Provide information and referral to individuals who contact CLW via electronic methods such as e-mail, phone, and fax on disability related topics and CLW's programs and services. + Other duties as assigned. + Complying with the requirements of State policies, including those concerning human resource components of security protocols of agency information security and confidentiality policies for state business purposes. + Coding, computing, verifying, posting, reconciling, and/or updating standard documents. + Other duties as assigned. **Knowledge, Skills, and Certifications:** To qualify for this position, the candidate must have training, education, or experience in office and administrative support work that demonstrates: + Competency in applying a solid knowledge of the principles and practices of quality customer service to perform complex work in assisting the public with applications, fees, examinations, or social services that have an eligibility or compliance component; and + The ability to use independent decision-making on the appropriate processes to follow, information to process, and actions to take in accordance with standard procedures + Knowledge of disability issues, and understanding/commitment to Independent Living philosophy and consumer direction. + Good communication, interpersonal, organizational and follow-up skills. + Ability to work alone and with others in a team + Able to ask for help/guidance when needed. + Maintain strict confidentiality . **Minimum Qualifications:** + Bachelor's degree in social work, business administration, human services, or other related field, or equivalent experience, required + 1-3 years of experience in working with aging and/ or disability populations, in the areas of aging and/or disabilities services, community health, nursing home or hospital discharge planning. + Excellent verbal and written communication skills, including sensitivity, listening, problem solving, and teamwork. + Ability to speak, read, and write English clearly and concisely. + Eastern Standard Time Location is preferred. + Employment is contingent on completion of a background check. + Own telephone, computer, wireless internet (Wi-Fi) and electronic equipment. + Must have access to a working PC with Windows 10, Antivirus Protection: Windows Defender and Windows Firewall; or Mac with Apple OS X 10.13, Antivirus Protection: Sophos; and personal mobile device to use for this job. **Location:** **The work will be performed remotely between the hours of 8am-4:30pm Eastern Standard Time.** Compensation for roles at Staffing Solutions Organization LLC (SSO) varies depending on a wide array of factors including, but not limited to, the specific office location, role, skill set, and level of experience. As required by applicable law, SSO provides a reasonable range of compensation for this role. In addition, SSO provides a range of benefits for this role.
